Unleashed MMA, LLC Student or Participant hereby represents that he/she is physically fit to take the prescribed course of instruction or voluntary participation and that no oral or written representations have been made to him/her except as expressly written in this agreement.
Student or participant further acknowledges the existence for the potential of personal injury as a result of participating in exercise and/or an activity such as Mixed Martial Arts, Boxing, Fitness Kick-Boxing, Brazilian Jiu-jitsu, Grappling, Wrestling, Judo, Personal Fitness Training, Strength & Conditioning and more and that he/she is assuming this risk without liability to school or instructors, student instructors, volunteer instructors and club managers by executing this agreement and participating in said course of instruction.
I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY AGREEING TO IT, AND HAVE AGREED TO IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY AGREEMENT TO BE COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
